Institutional Animal Care and Use Committee: Step 1

Step 1 - Complete the Request for Use of Animals Form

To begin the process of obtaining approval for your project, complete one of the following forms and submit it 45 days prior to the onset of your project.

• Research Request for Use of Animals Form (request for animal use in research)

• Education Request for use of Animals Form (request for animal use in courses)

No use of vertebrate animals for research or education is permitted until approval is granted by the IACUC. Please send completed forms to the chair of the Institutional Animal Care and Use Committee (IACUC), Dr. Leah Stiemsma, in the Natural Science Division.
