Planning, Operations, and Construction (POC) provides oversight and administrative support for the Department of Design and Construction and the Department of Facilities Services in the areas of personnel, finance, database systems, information technology, and design.

Project Request Forms

There are various types of facility and space modification project requests, and choosing the proper request form is important to ensure your project receives the appropriate and timely attention it deserves.

Types of Request Forms

UMC Request Form

Typically, larger projects (>$75,000) that fundamentally change the use of a space – including purchases of furniture, fixtures, finishes, and artwork (primarily applies to public-facing spaces, not to private offices) – require a renovation, remodel or construction are projects that may need University Management Committee (UMC) approval. The criteria are outlined here.

Non-UMC Request Form

Typically, for requests that do not meet any of the UMC criteria detailed above but require POC assistance – such as painting an office (or full wall), installing window shades, or refreshing furniture or signage – requesters should complete the Non-UMC Request Form.

Maintenance Request Form:

Typically, small maintenance and routine repair tasks such as paint touch-ups, changing lightbulbs, cleaning carpet, patching a wall, or fixing a door knob are handled by the Department of Facilities Services (DFS). Click the DFS page button below to complete a Maintenance Request Form.

What to Expect After Submitting a Form

Once your UMC or Non-UMC form is submitted, POC will assign your project an ID number and a project manager (PM), who will reach out if additional information is required. While we seek to address matters as quickly as possible, it can take several weeks, or even months (depending on project scope), for the PM to complete a feasibility review – conducting an impact analysis, scope of work verification, and rough order-of-magnitude (ROM) cost estimate. If your project does not already have approved funding, a source of funding will need to be identified during this stage. At some point in this process, it may be determined that your project is not viable. Decisions about project viability will be communicated to the requester. If your project is initiated, you will receive periodic updates from the PM.
