||Externs returning to a previous placement should complete this form to seek approval
for the placement.
|Fall or Spring Registration Packet
||All externs seeking externship credit during the Fall or Spring terms should complete
|Summer Registration Packet
||All externs seeking externship credit during the Summer term should complete this
|Confirmation of Externship Offer
||All externs must ask their Field Supervisor to complete this form. Students may also
request an e-signature form.
|Request to Work Remotely
||This form is intended for students who are requesting permission to work remotely
while earning externship credit.
|Externship Registration Deadline Extension Request
||Students who are unable to return their completed paperwork by the deadline due to
external, extenuating, and unforeseen circumstances should complete this request by
5:00pm on the last day of the add/drop period.
|Petition to Register Past the Registration Deadline
||This form is intended for students initiating registration for an externship after the deadline to register (the last day of the add/drop period). Note that petitions
must be approved by the Director of Externships, the Assistant Dean of Clinical Education,
and the Vice-Dean for Academics.
