Forms Purpose
Understanding of Externship Guidelines All externs should complete this form at the beginning of the externship registration process.
Continuing Placement Statement Externs returning to a previous placement should complete this form to seek approval for the placement.
Fall or Spring Registration Packet All externs seeking externship credit during the Fall or Spring terms should complete this form.
Summer Registration Packet All externs seeking externship credit during the Summer term should complete this form.
Confirmation of Externship Offer All externs must ask their Field Supervisor to complete this form. Students may also request an e-signature form.
Request to Work Remotely This form is intended for students who are requesting permission to work remotely while earning externship credit.
Externship Registration Deadline Extension Request Students who are unable to return their completed paperwork by the deadline due to external, extenuating, and unforeseen circumstances should complete this request by 5:00pm on the last day of the add/drop period.
Petition to Register Past the Registration Deadline This form is intended for students initiating registration for an externship after the deadline to register (the last day of the add/drop period). Note that petitions must be approved by the Director of Externships, the Assistant Dean of Clinical Education, and the Vice-Dean for Academics.
