All externs must ask their Field Supervisor to complete this form. Students may also
request an e-signature form. |
| Request to Work Remotely |
This form is intended for students who are requesting permission to work remotely
while earning externship credit. |
| Externship Registration Deadline Extension Request |
Students who are unable to return their completed paperwork by the deadline due to
external, extenuating, and unforeseen circumstances should complete this request by
5:00pm on the last day of the add/drop period. |
| Petition to Register Past the Registration Deadline |
This form is intended for students initiating registration for an externship after the deadline to register (the last day of the add/drop period). Note that petitions
must be approved by the Director of Externships, the Assistant Dean of Clinical Education,
and the Vice-Dean for Academics. |
| Petition to Reduce Externship Units |
This form is intended for students seeking a reduction in externship units after the add/drop period ends. Note that petitions must be approved by the Director of
Externships, the Assistant Dean of Clinical Education, and the Vice-Dean for Academics. |
| Petition to Increase Externship Units |
This form is intended for externs petitioning to increase the number of externship
units they are registered for after the add/drop period has passed. Please note that
increases are rarely permitted and require a showing of substantial hardship. |
| Petition for Exception to Externship Guidelines |
This form is intended for students who wish to seek an exception to Externship Guidelines
as described in the Understanding of Externship Guidelines. If one of the petition forms above is applicable to your situation, please fill
