Services Rendered or Scholarship Payment Rendered to a Non-resident Alien

1. Foreign National Information Form.

2. Printed Copy of I-94 from I-94 Website and I-94 Travel History. Passport, Visa, and if F-1, I-20, Certificate of Eligibility of Nonimmigrant, or I-688B, Employment Authorization Document (EAD) OR if J-1, DS-2019, Certificate of Eligibility for Exchange Visitor Status.

3. Visitors Claiming Tax Treaty Benefits. (If visa holder is from a Tax Treaty country and claims benefits, they must have an SSN or ITIN to receive Treaty Benefits.)

a) Form 8233 (Student will need to come in to the Payroll Department to complete this form.)
