NOTE: the PeopleSoft RDS form is ONLY to be submitted if you will be downloading Social Security numbers for valid uses, e.g. government reporting - this is not: 1) a PeopleSoft Finance access form or 2) a form required to obtain a Purchase Card or 3) a form to allow convenience use of a report that contains SSN but you will not process SSN data.
